The Long Island Family Dental In-Office Benefit Plan is an alternative for individuals, couples, families, and businesses who want excellent dental coverage without the barriers of traditional insurance. It is not dental insurance and is exclusive to our office.

In-Office Benefit Plan Cost
Individual Plan
$440 /per year
Each additional family member
$350 /per year
Comprehensive Periodontal Plan (add-on)*
+$200 /per year
*For patients who have completed advanced periodontal treatment. Includes 4 healthy cleanings per year.
What's Included
- Two healthy cleanings per year (prophylaxis D1110 only; excludes scaling and root planing and full mouth debridement)
- Two check-up exams per year, with radiographs once per year
- Oral cancer screening
- Periodontal screening
- Fluoride treatment as needed (no age restrictions)
- One emergency exam per year
- 15% savings on general dentistry and routine restorative treatment: fillings, crowns, and in-office teeth whitening (excludes surgical implant placement)
- Invisalign consultation, plus 15% savings on Invisalign full cases over $6,000
Why Preventive Care Matters
Regular cleanings are the cornerstone of long-term oral health. Catching decay, broken fillings, and gum disease early helps you avoid larger problems like root canals, tooth loss, crowns, and bridges.
Terms & Conditions
- Membership runs one year from registration. Unused services do not carry over.
- Plan fees are due at enrollment. Discounted treatment fees are due when treatment starts.
- Fees are non-refundable, and the plan is non-transferable.
- Cannot be combined with dental insurance, other discounts or dental plans, professional courtesy, workers' compensation, hospital charges, or auto medical claims.
- Benefits apply only at our practice. Outside specialist referrals are not covered.
- Procedures in progress or performed before or after the membership period are excluded.
- An $80 fee applies to missed or cancelled appointments without 24 hours' notice.
- Our practice reserves the right to terminate membership with 30 days’ written notice if the account becomes delinquent or the patient fails to comply with the plan’s terms and conditions.
- Plan fees may be modified annually.
